Job Description

PURPOSE OF POSITION:
Under direct supervision of a licensed registered nurse, the HPCT performs activities and procedures within the appropriate scope in direct/indirect care of patients and participates as a member of the nursing/medical health care team in implementing appropriate treatment regimen for the assigned patient population.

AREAS OF RESPONSIBILITIES:

All Areas of Renal Dialysis, including Inpatient, Outpatient, Isolation, Bedside and Emergency Department.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

The responsibilities include, but are not limited to:

  • Provides care and communicates with patients considering their age and developmental status
  • Provides assistance with activities of daily living and clinical care, including but not limited to, changing simple dressings, Blood Glucose monitoring via finger stick, taking vital signs, and obtaining 12 EKG.
  • Acknowledges and respect patients' right to privacy while receiving care.
  • Demonstrates respect for patients/families by explaining procedures and maintaining confidentiality of medical records.
  • Prepares patient for examination or procedures by obtaining and recording vital signs
  • Prepares and test equipment for safe operation and transports various equipment and supplies.
  • Reports unusual observations including any change in patient's condition/behavior to professional staff or primary nurse.
  • Ensures safe escort by obtaining handoff report, securing patient care apparatus such as IV tubing and Foley catheter/bag, and providing privacy.
  • Collects and labels specimens at the point of care using two patient identifiers.
  • Performs assigned errands including transporting specimens to pathology or other areas.
  • Documents appropriate information on designated area of the medical record.
  • Adheres to standard infection control precautions.
  • Consistently performs hand hygiene before and after patient care/contact
  • Performs blood sampling via venipuncture accordingly to hospital policy.
  • Performs finger stick blood glucose measurement and documents outlier codes consistently.
  • Completes assignment in allotted time.
  • Reports to Charge Nurse/primary nurse before leaving assigned area/clinic.
  • Reports safety hazard and initiates action to eliminate hazards or refers to R.N.
  • Maintains current Cardiopulmonary Resuscitation certification status as per hospital policy.
  • Prepares dialysis machines for treatment, including: priming of dialyzer, performing machine alarm check and confirming machine pH, conductivity and in accordance with policies and directives.
  • Monitors equipment for proper functioning and ensures that machine conductivity and temperature are maintained at all times.
  • Ensures that sufficient par levels of supplies are available and.
  • Inspects and monitors arteriovenous (AV) fistula or AV graft and reports problems to RN.
  • Initiates hemodialysis treatment after RN's assessment.
  • Obtains and documents pre and post treatment, vital signs and weight;
  • Collaborates with assigned RN in individualized hemodialysis treatment plan, as appropriate, in order to meet patient's continued health need
  • Cannulates the AV fistula or AV graft as per policy and provider's prescription.
  • Observes, monitors and record patient's vital signs during treatment; identifies abnormal changes and reports to assigned RN immediately
  • Administers saline under the direction of RN during hypotensive emergency.
  • Terminates hemodialysis treatment and disconnects patient from machine; carefully removes cannulation needles to ensure minimal blood loss.
  • Strips and cleans machine with appropriate disinfectant(s) after each treatment and sets machine for use by the next patient.
  • Sets up and breaks down dialysis machines and other relevant equipment.
  • Daily rotational schedule for standby/on-call for emergent dialysis and follow the protocol.
  • Monthly rotational Sunday assignment for the water system and machines disinfection.
  • Participates in the facility and unit Quality Assurance/Quality Improvement (QA/QI) initiatives.
  • Demonstrates knowledge of emergency situations/disaster response.
  • Maintains annual mandatory education; attends scheduled education programs.
  • Conforms to policies governing patient abuse, patient rights, suicide/ligature risk screening.
  • Wears valid Kings County Hospital Center identification card visibly at all times.
  • Observe Department's dress code and use of private phone policy.
  • Provides proper notification for absence/tardiness and confirms to time and leave regulation.
Minimum Qualifications
1. A High School diploma or a G.E.D; and
2. Successful completion of a Hemodialysis Patient Care Technician training course as evident by a valid certificate of completion or equivalent documentation; and
3. Certification issued by a national commercially available or state recognized Hemodialysis Patient Care Technician program must be obtained within 18 months of being hired into this title; and
4. Completion of an approved facility-based training program in the performance of basic life support procedures including certification in Cardio-Pulmonary Resuscitation.

Special Notes:
1. Nationally available certification programs endorsed by the Centers for Medicare and Medicaid Services (CMS), Code of Federal Register - 494.140(e), include those of the Nephrology Nursing Certification Commission (NNCC), the Board of Nephrology Examiners Nursing and Technology (BONENT), and the National Nephrology Certification Organization (NNCO).
2. An in-house certification and/or training program may be administered / conducted to assess and develop individuals assigned to this title; however, such internal efforts do not replace the CMS requirement that hemodialysis patient care technicians be nationally, or state certified as outlined in 3 above.
3. This is a physically demanding position requiring employees to lift, carry, push and pull dialysis machines, related equipment and/or supplies which vary in weight and size, as well as stand and walk for extended periods of time. Employees must be able to carry out other physical demands that are essential to the job.
4. This title is for use in outpatient end stage renal disease (ESRD) facilities only.
