Job Summary:

Responsible for assisting the social workers with the coordination of patient aftercare/discharge planning. The Discharge Planner will communicate and work with external contacts such as: community resources, referral sources, family, social service agencies, and insurance companies to assist with the discharge process.

Job Responsibilities:
  • Reviews/modifies information as necessary for discharge planning
  • Participates in family and provider meetings to develop a continuing care plan for the patient leaving treatment
  • Provides clinical information to external contacts in preparation of discharge.
  • Identifies resources that will accepts patients' insurance.
  • Provides clinical information to payers.
  • Attends treatment team meetings.
  • Contributes to process improvement of the role.
  • Performs other duties as assigned by supervisor
Job Requirements:
  • Bachelor's degree is preferred.
  • Familiarity with psychiatric patients, substance use clients, including adolescents.
  • Familiarity with levels of care and clinical criteria for appropriate aftercare placement.
  • Familiarity with collateral contacts, community resources and placement/referral processes.
  • Strong organizational skills.
  • Strong interpersonal skills.
  • Strong time management skills.
  • Goal oriented and accountable.
