We invite applications for a specialist in Internal Medicine or Family Medicine with Complex Care experience to support the Baltimore Service Area.

In collaboration with the Kaiser Foundation Health Plan of the Mid-Atlantic States, we provide high-quality, patient-centered health care. Through this partnership, our physicians are part of an industry-leading health care delivery model, having access to advanced technological tools and receiving comprehensive practice support.

MAPMG physicians are salaried, without the pressures of alternative payment models or fee-for-services.Our members have easy access to specialists and medical records, thus allowing you to treat the whole patient.

What You Can Expect:

  • Provide integrated medical care with the support of dedicated Complex Care case managers/social workers and research RN.
  • Perform weekly huddles with Complex Care team including research RN, case manager and social worker.
  • Provide KP cell phone to enrolled patients for direct access.
  • Primarily provide outpatient care with occasional home visits for patients that qualify.
  • Provide social visits to Hospital, SNF and Long-Term Care facilities as needed or required.
  • Regular review of CCP referrals with timely coordination of patient enrollment.
  • Perform routine outreach to enroll patients that qualify to the program.
  • Coordinate timely follow up for Hospital and SNF discharges.
  • Multidisciplinary/specialist care coordination.
  • Annual completion of CITI training to maintain IRB certification.
  • Required participation in monthly departmental & quarterly regional meetings.
Minimum Requirements:
  • MD or DO
  • Board Certified or Eligible in Internal Medicine or Family Medicine
  • Five years outpatient experience or five years of experience in hospital medicine
  • Currently licensed or able to obtain licenses in the District of Columbia, Maryland, and Virginia
  • Work visa sponsorship is available for eligible applicants.
