We are seeking a skilled Complex Care Social Worker to join our team at Grace Medical Center. As a key member of our interdisciplinary team, you will play a critical role in facilitating discharge planning for patients with complex needs.
Using a multi-step escalation protocol, you will identify patients with barriers to effective discharge and work collaboratively with the team to develop and implement a plan of care. Your expertise will be essential in monitoring high-cost patients and identifying early intervention opportunities to effect a timely and cost-saving discharge.
Responsibilities- Develop and implement discharge plans for complex patients
- Collaborate with the interdisciplinary team to identify and address patient needs
- Monitor high-cost patients and implement early intervention strategies
- Provide leadership and creativity in developing programs and projects to enhance care management
- Master's degree in social work or related field
- Current/Valid license in the State of Maryland (LCSW-C/LCPC)
- Minimum of 3 years of experience in social work or related field
