MAIN FUNCTION:

The Clinical Staff Pharmacist is responsible for receiving, reviewing, and interpreting physicians orders, dispensing medications for patients, and providing drug and related information to the Medical and Nursing Staff.

This position reports to the Director, Pharmacy and operates within established organizational and departmental policies and procedures.

ESSENTIAL JOB FUNCTIONS:

  • Perform all the technical and professional functions related to the dispensing of any medications.
  • Assess the clinical appropriateness of medication orders based on the patients diagnosis, clinical condition, height, weight, allergies, concurrent therapy and organ function.
  • Works in conjunction with the Pharmacy Clinical Manager to provide clinical and educational services to the medical and nursing staff. Directs pharmacy student activities as assigned.
  • Advise patients and families on the use of various medications including dosage, side effects and composition.
  • Provide clinical pharmacy services including, but not limited to, pharmo-kinetic dosing, renal dosing adjustments, culture and sensitivity reviews, therapeutic drug monitoring and drug use reviews.
  • Maintain proper operation of equipment and supplies in assigned work areas.
  • Ensure adequate record keeping involving all controlled substances in accordance with all federal, state and local laws.
  • Ensure secure storage of narcotics and other controlled substances on a regular basis.
  • Rotate shifts as necessary to maintain 24 hour department function.
  • Use computer according to established guidelines, maintaining strict confidentiality at all times. Accurately enter drug orders on to patient profile via computer order entry.
  • Demonstrate competency in mixing/compounding sterile medications, including large volume parenteral solutions, piggybacks, total parenteral nutrition and chemotherapy.
  • Routinely document identified medication process errors and assist with the follow up of MIR & ADR reports as requested.
  • Maintain established hospital and departmental policies and procedures, objectives, confidentiality, quality improvement program, compliance, safety, infection control and environmental standards.
  • Conduct and evaluate medication histories, assess compliance and suggest modifications so as to achieve desired outcomes and control costs; instruct patients in the proper use of prescribed drugs and make Patient Care Rounds with physicians to evaluate patient progress.
  • Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.
  • Suggest therapeutic alternatives to providers when sub-optimal therapies have been identified.
  • Maintains and updates personal drug knowledge to ensure accurate provision of pharmaceutical care for general disease states; maintains advanced practice base through the avenues of publications, presentations, educational boards, and professional consultation, association membership, and certification.
  • Meet annual clinical and hospital competency and retraining requirements.
  • Attend meetings as required.
  • Perform other functions/duties as requested.
Qualifications:

MINIMUM REQUIREMENTS:

  • Graduate of an accredited school of pharmacy required
  • Current registration as a Registered Pharmacist in the Commonwealth of Massachusetts required
  • Previous hospital experience preferred
  • Demonstrated effective communication skills required
  • Demonstrated ability to be self-directed, motivated, and capable of supervising supporting personnel required
