Independent Support Coordinator

JOB DESCRIPTION

TNCSA is a provider of Independent Support Coordination (ISC) services for the Department of Intellectual and Developmental Disabilities. The Independent Support Coordination Program assists individuals with intellectual and development delays to live as independently as possible within their communities through the provision of community-based services.

Part time position. ** Job is remote; however, must reside in the Nashville area due to assignment of case load.** Must be available to work during the day, Monday - Friday.

RESPONSIBILITIES

  • Case management services for individuals with developmental and intellectual disabilities,
  • Conduct meetings, writing individual support plans and submitting those plans to DIDD in order to request services,
  • Monthly monitoring of services and completion of vital paperwork.

MINIMUM QUALIFICATIONS

  • Bachelor’s degree – Preferred field social work, psychology, sociology or related field.
  • One to two years of work related experience.
  • ISC or DIDD experience and/or certification a plus.
  • Excellent written and verbal communication skills.
  • A valid motor vehicle operator’s license is required. Must have reliable transportation.
  • Must authorize the release of any investigative and criminal records obtained by the Federal Bureau of Investigation and the Tennessee Bureau of Investigation to the Tennessee Community Services Agency.
