Gould Hospital Work Type: Full Time Hours Per Week: 40.00 Work Schedule: 8:00 AM to 4:30 PM *This is a Hybrid position* Summary: Oversee clinical care to patients with complex care needs through telephonic and in person contact, with focus on chronic disease and transitions of care. Serves in an expanded nursing role to collaborate with the Medical Team to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. Integrates evidence based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare. Responsibilities: Effectively communicates needed information to Providers. · Compiles, analyzes, and interprets data per business needs. · Manages / maintains electronic health records in accordance with confidentiality standards and policies. · Directly leads work efforts of subordinates and/or peers, guides and advises co-workers. · Attends and participates in meetings. · Provides logistic, financial, emotional support and guidance to patients. · Works to meet patient's psychological, emotional, or physical needs. · Identifies and advocates for improvement to processes within the department and organization when identified. · Maintains thorough understanding of department's key performance indicators and strives to achieve them. Manages various aspects of staff performance and development and maintains a safe environment complying with NLH policies and procedures; reports and directly addresses environmental hazards and violations of patient safety policy and/or protocol when involved or observed. · Other Duties Competencies and Skills + Behaves with Integrity and Builds Trust: Acts consistently in line with the core values, commitments and rules of conduct.
