JOB SUMMARY/PURPOSEPerforms the duties of Pharmacist as prescribed by state and federal statutes as well as SHS policies. Prepares and dispenses medication; assesses clinical status of patients (neonates, infants, pediatrics, adolescents, adults and geriatrics). Provides drug information to the medical team. Educates patients.DEPARTMENT DESCRIPTIONHome Infusion is staffed by pharmacy staff that are knowledgeable in quality infusion and enteral care of patients receiving care in the home setting.EXPERIENCE/EDUCATION/QUALIFICATIONSCurrent unencumbered Oregon Pharmacist Licensure required.Previous hospital pharmacy experience preferred.All job offers are contingent upon the candidate passing a pre-employment/post offer drug screen.GSR Home Infusion:Healthcare Provider Level BLS required within three (3) months of hire.GSRMC Specialty Pharmacist (if applicable):Pharmacy experience in the specialty disease states required.KNOWLEDGE/SKILLS/ABILITIESCommunication - Effective written and oral communication skills to explain complex issues, exchange information between team members, and tactfully discuss issues. Ability to perform group presentations and listen to and understand complex information and ideas.Computer Literacy - Knowledge of electronic equipment and computer hardware, software, and applications. Ability to operate applications, enter data, and process information.Social Perceptiveness - Ability to work with a diverse population from pediatrics to geriatrics. Basic understanding of age-related differences in caring for and/or communicating with patients and caregivers. Possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles and ages.Team Building - Ability to work, function and communicate on a multi-disciplinary team. Possess the knowledge and skills to develop constructive and cooperative working relationships with others and maintain them over time. Demonstrates teamwork. Possess good judgment and decision making skills.Specialty Pharmacist (if applicable):Ability to read, understand, translate and enter prescription orders into the pharmacy management system.Knowledge of rules and regulations affecting specialty pharmacy. Stays current on rules and regulations.Knowledge and training in the specialty disease states.
