Position Summary:

We are seeking a highly motivated and organized Service Coordinator for our Jackson Heights location. As a Service Coordinator you will be responsible for triage services provided under the HIV Supportive Services Program. This includes ensuring client eligibility and coordinating centralized intake, interdisciplinary team case conferencing, and making additional internal or external referrals as needed.

Job Responsibilities:

+ Adheres to AIDS Institute requirements for the provision of HIV Supportive Services program.

+ Conduct intake assessment and screen clients needs and work closely with the case manager and peer navigator to ensure implementation of service/care plan an/or case conference.

+ Develops service plans that incorporate goals and objectives, frequency and duration of the services, HIV specific health and nutrition topics.

+ Work closely with the Program Manager for Support Services to ensure appropriate services delivers to the program clients including conduct HIV Support Services program orientation, screening/re-screening, and intakes including the reasons for participation in program services.

+ Acts as point person with HIV Support Services program staff to coordinate activities including to assist the Program Manager for Support Services to ensure adherence to the protocols and standards of Department of Support Services which in line with the agency.

+ Provide psychoeducation support and nutrition health educations groups to the program clients.

+ Work closely with the case manager and peer navigator to coordinate the purchase and distribution of vouchers that meet program guidelines.

+ Recruit and negotiate with vendors that will accept Apicha Food Voucher.

+ Maintain financial and program variances of the program.

+ Assist the Program Manager for Support Services to coordinate and attend all required training sessions and meetings (e.g., care planning, ad hoc, training, and supervision).

+ Work closely with Apicha CHC staff and other service providers to coordinate clients' care.

+ Manage the program clients satisfaction and exit surveys; complete the reports and represent the results.

+ Manage and monitor the program key deliverables to the clients and meet the programs benchmark.

+ Maintain accurate records on the services and activities such as attendance, registration, clients interactions and other required documentation according to the protocols and standards of Apichas Department of Support Services in designate database and reporting structures.

+ Write and submit reports adhering to deadlines.

+ Work as a part of the team for quality assurance and continue quality improvement (CQI) tasks.

+ Communicate effectively with clients, families, and the public to build trusting relationships across a broad range of socioeconomic and cultural backgrounds.

+ Represent the agency at various venues to promote agency services.

+ Other activities as assigned.

Qualifications:

+ BA/BS in related field with two (2) years of qualifying experiences required. An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience may be considered.

+ Experience working with target populations strongly preferred. The target populations: HIV/AIDS who are virally unsuppressed, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis.

+ Fluency in Spanish and/or Asian languages a plus.

+ Excellent organization, written, verbal, and interpersonal communication skills in English required.

+ Consistent team player capable of effectively operating within the organizational structure.

+ Appreciation of and commitment to Apichas mission and values required.

+ Excellent customer service skills to assist in resolving member complaints.

+ Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC.

+ Strong verbal and written communication skills for both internal and external audience.

+ Computer literacy ability to learn and adapt to different reporting platforms within required time frames.

+ Ability to work effectively in stressful environment.

+ Ability to work effectively in both a collaborative as well as individual setting.

+ Ability to work flexible hours including some evenings and weekends.
