Call Center Specialist,

CareCloud is seeking an experienced Call Center Specialist, LPN or RN to join our team on a full-time basis. This position entails planning, facilitating, enhancing, and coordinating a wide array of services across the care continuum for our Medicare demographic. Ideal candidates will be adept in providing telephonic patient care and will play an integral role in our clinical team, applying their healthcare knowledge, skills, and problem-solving abilities to oversee a monthly patient roster and ensure comprehensive care for everyone.
Essential Duties and Responsibilities:
- Provide case management services to patients with chronic or complex conditions
- Assess patients current health status, treatment plan, services, short/long-term goals, provider options
- Develop a care plan using judgment and assessment skills
- Provide patient education and ongoing monitoring of care plan to evaluate effectiveness, modifying as needed.
- Coordinate services required to enhance wellness and manage chronic conditions, improve quality of life, and decrease emergency room visits and avoidable hospital admissions
- Advocate for patients, act as liaison, escalate information as appropriate
- Complete documentation/reporting in accordance with company procedure
- Administrative, reporting, documentation tasks as assigned
- Oversee the Chronic Care Case Management team, including approval of timesheets and PTO
- Participate in the recruitment process for new Chronic Care Case Management team members
- Perform supplementary assignments as needed.
Required Knowledge, Skills and Abilities:
- 1+ years of experience in the healthcare industry; preferably in internal or family medicine clinic.
- Experience working with chronically ill patients preferred.
- Ability to maintain confidentiality and follow HIPAA guidelines.
- Ability to handle difficult and sensitive conversations.
- Type and talk at the same time.
- Comfortable learning and using new EHR platform.
- Proficient in MS Windows, Word, Excel and Outlook.
- Excellent written and verbal communication skills.
- Must be a team player.
- Present professional and friendly demeanor to client and colleagues.
- Accomplishing tasks with minimal supervision.
Bilingual Proficiency:
- Proficiency in both English and Spanish is required.
- Ability to communicate effectively in both languages, including speaking, reading and writing to meet patient needs.
Education and Experience:
- 1+ year face-to-face patient healthcare experience
- 1+ years of experience in the healthcare industry; preferably in a providers office, facility or
medical billing/RCM company
- Registered Nurse or LPN License (required)
- Previous experience with multiple EHR software and platforms

Compensation: $27.00 - $33.00 per hour
