The pharmacist assists prescribers in caring for patients on anticoagulation therapy to achieve desired outcomes. What you will Do: * Provides oversight and guidance for the Anticoagulation Clinic Service. * Performs drug regimen / laboratory data review and adjusts anticoagulation therapy, when appropriate, to maximize outcomes for all Anticoagulation Clinic Service patients. * Provides appropriate patient monitoring to ensure that desired outcomes are met by following Anticoagulation Clinic protocol and Chest Guidelines * Educates patients and families regarding anticoagulation therapy. * Maintains organized documentation on recommendations, interventions, follow-up actions and outcomes in Coagclinic and Webcare. * Communicates effectively with prescribers referring patients to the Anticoagulation Clinic. * Maintains competency in anticoagulation care. Serves as an active patient advocate regarding anticoagulation therapy Uses a team approach to working with staff and others Hours/Schedule: PRN; as needed. Minimum Qualifications: * At a minimum, a BS in Pharmacy from an accredited college of pharmacy. PharmD preferred. * Iowa Pharmacist Licensure, either by examination or by reciprocity. * Demonstrated competence in providing direct patient care. * Previous anticoagulation management experience preferred. * Self-motivated, detail-oriented individual able to work with minimum supervision. * Good communication skills. * Leadership skills. * Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
