WHAT YOU'LL BE RESPONSIBLE FOR
Reporting to the Enhanced Health Services Director, this position:
Manages and leads the Enhanced Care Management (ECM)/In Lieu of Services (ILOS) Unit, acts as a subject matter expert, and provides guidance related to the implementation and management of the ECM benefit and ILOS
Provides management oversight related to ECM/ILOS Unit and departmental operations
Manages, supervises, mentors and trains assigned staff
WHAT YOU'LL NEED TO BE SUCCESSFUL

Knowledge of:
Principles and practices of case management
Community-based organization and/or medical office practices and processes, including those related to workflow design and improving the patient/consumer experience
Principles and practices of community-based service delivery to at risk populations
Ability to:
Analyze, interpret and apply legal, regulatory and contractual language, policies, procedures and guidelines, and legislative and regulatory directives
Train, mentor, supervise, and evaluate the work of staff, promote an atmosphere of teamwork and cooperation, and motivate staff to achieve goals and objectives
Organize and prioritize the work of others, delegate effectively, and follow up on work assignments
Develop work plans and workflows and organize and prioritize ECM/ILOS Unit activities
Manage the development and implementation of policies, procedures, projects, systems, and programs, including integrated programs for community based enhanced care management and the patient centered medical home model
Education and Experience:
Bachelor’s degree in Healthcare Administration, Nursing, Behavioral Health, Social Science or a related field
A minimum of sixyears of professional level outpatient medical, community health service delivery, behavioral practice management, or related healthcare experience, including some lead or supervisory responsibility (a Master’s degree may substitute for two years of the required experience); or an equivalent combination of education and experience may be qualifying
OTHER INFORMATION
We are in a hybrid work environment and we anticipate that the interview process will take place remotely via Microsoft Teams.
While some staff may work full-time telecommuting schedules, attendance at quarterly company-wide events or department meetings will be expected.
In-office or in-community presence may be required for some positions and is dependent on business need. Details about this can be reviewed during the interview process.

The actual compensation for this role will be determined by our compensation philosophy, analysis of the selected candidate's qualifications (direct or transferrable experience related to the position, education or training), as well as other factors (internal equity, market factors, and geographic location).
