Employment Type:Part timeShift:Day ShiftDescription:Position Purpose:The Anticoagulation Clinical Staff are responsible for the Anticoagulation Clinic Service. The pharmacist assists prescribers in caring for patients on anticoagulation therapy to achieve desired outcomes.What you will Do:Provides oversight and guidance for the Anticoagulation Clinic Service.Performs drug regimen / laboratory data review and adjusts anticoagulation therapy, when appropriate, to maximize outcomes for all Anticoagulation Clinic Service patients.Provides appropriate patient monitoring to ensure that desired outcomes are met by following Anticoagulation Clinic protocol and Chest GuidelinesEducates patients and families regarding anticoagulation therapy.Maintains organized documentation on recommendations, interventions, follow-up actions and outcomes in Coagclinic® and Webcare.Communicates effectively with prescribers referring patients to the Anticoagulation Clinic.Maintains competency in anticoagulation care. Serves as an active patient advocate regarding anticoagulation therapy Uses a team approach to working with staff and othersHours/Schedule: PRN; as needed.Minimum Qualifications: At a minimum, a BS in Pharmacy from an accredited college of pharmacy.
