Under the direction of the Health Care for the Homeless (HCH), Director, the Case Manager will work independently to provide coordination of HCH services, physical health, mental health and chemical dependency treatment for HCH patients and will address the problems and needs associated with the conditions of homelessness.
II. Essential Responsibilities

  • Maintains appropriate patient records for continuity of care, treatment planning, and patient tracking; prepares and submits all required patient and activity reports;
  • Ensures proper patient demographic information is entered into computer systems at each visit if applicable, for homeless patients;
  • Refers patients to appropriate community and medical services if not eligible for HCH program;
  • Provides case management services to patients, with special attention and understanding of the needs and circumstances related to homelessness;
  • Carries out Case Management within the context of on-going needs, care and/or treatment planning, and monitoring;
  • Develops and determines a specific, comprehensive, individualized care/treatment and service plan determining formal and/or informal, short and/or long term, using strengths model for patients;
  • Help patients with life skills training;
  • Refers or transfers patients to necessary services, treatments and formal/informal support systems;
  • Assists patients in gaining access to Federal, State, local, and private programs that provide or financially support the provision of medial, social, educational, or other related services;
  • Works in conjunction with outside agencies and service providers to facilitate case coordination and appropriate information sharing, and to assists in coordination of patient support services to meet the basic needs of homeless individuals by:
    • Helping individuals obtain safe, affordable, and permanent housing;
    • Assisting with access to mental health and substance treatment services;
    • Providing crisis assistance;
    • Identifying educational and employment options;
    • Developing a social support network;
    • Assisting to access daily needs such as food, clothing, hygiene items.
  • Conducts ongoing evaluation of patient progress and needs, and helps patient plan to transition from one type of setting or service program to another;
  • Coordinates and monitors the activities of the homeless individual's care/treatment plan with the primary responsibility to log the treatment plans progress, to provide individual supportive services, to ensure immediate changes are made in the treatment plan as the patient's needs change, and to advocate for the patient's rights and preferences;
  • Intercedes on behalf of patients to ensure equity and appropriate services;
  • When necessary, accompanies patients to appointments in the HCH van and advocate on behalf of the patient;
  • Practices safety protocols and de-escalation techniques at all times;
  • Sets and adheres to boundaries with patients;
  • Insures safety by informing the Project Director and/or appropriate HCH office staff of Case Managers location during street outreach and other patient centered activities;
  • Performs street outreach as an effort to approach and engage homeless persons with the objective of developing an ongoing relationship of trust; attempting to enroll patients not using normal services;
  • Offers alternatives to the homeless individual's current living situation;
  • Informs individuals of the availability of health care (HCH), substance abuse services, and assists them in accessing needed services and provide ongoing emotional support and follow-up;
  • Networks with and maintains relationships with outside agencies and organizations to increase awareness, competence, and resources pertaining to the homeless population;
  • Provides monthly reports, as requested by Program Director;
  • Preforms responsibilities as assigned.
I. Education, Experience and Skills/Training
  • High School diploma or equivalent required; preferred Associate's degree or Bachelor's Degree in Social Services, Counseling, Social Work and two years of experience in a related field. In lieu of a degree, the incumbent must possess sufficient prior work experience in progressively more responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.
  • Ability to work and manage multiple priorities in a fast-paced environment to meet established goals and deadlines, and excellent communications skills, verbal, written, and listening;
  • Ability to work with homeless community, and the chronic mentally ill, substance abusers, and other persons who have co-occurring disorders that might have long-term mental illness and previous institutional history;
  • Ability to demonstrate sensitivity to the multicultural nature of the service population;
  • Knowledge of community resources;
