We provide services across 17 counties along the front range of Colorado.PASCO has worked tirelessly to bring accessible transportation, family caregiving, service expansions, and innovative legislation for over 30 years Built by advocacy, sustained by compassion.POSITIONI/DD Waiver CoordinatorPOSITION PURPOSETo support PASCO families, clients, and caregivers enrolled in I/DD medicaid waivers (DD, CES, SLS) while adhering to state and PASCO policy.KEY DUTIES AND RESPONSIBILITIESCoordinate and oversee Individualized Service Plans (ISPs)Collaborate with healthcare providers and social workers to ensure comprehensive careAdvocate for clients and ensure their needs are met in a person-centered approachStay informed on I/DD waiver regulations and updates to ensure complianceEmpower clients and families throughout the waiver process by providing guidance and supportCommunicates regularly with PASCO service leaders to ensure consistent messaging on the advantages that PASCO provides to clients and caregiversUpdates Client Relationship Management (CRM) system regularly to build upon networking opportunities and appropriate follow-upReceive, track, and audit incoming documentation from our clients and caregiversCreate and maintain client and provider schedules in electronic health records systemKEY MEASURES OF PERFORMANCEStrong communication and interpersonal skillsFeedback from individuals receiving servicesMaintaining compliance with due dates, adhering to PASCO and state policyRequired SkillsMeticulous attention to detailExceptional customer service aptitudeProficient time management/productivity abilitiesStrong communication skills within a team settingCapability to travel to clients' homes on a quarterly basis (required)A passion for collaborative teamwork and dedication to PASCO's Core ValuesPreferred skillsCase management experienceBilingual (reading, writing, speaking) in Spanish highly preferredGeneral working knowledge of Colorado's I/DD Medicaid waivers (DD, SLS, CES)Physical or Other Requirements:Medium detailed work:Access to a reliable transportation, either public transportation or an automobile with a valid Colorado driver's license and insurance.Requires walking or standing a significant amount of the time.Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.Requires stooping, kneeling, crouching, grasping, lifting, reaching, pushing.Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).JOB TYPE Full-timeLOCATION Hybrid: 2 day onsite (Wed/Th) in our Lakewood offices with other days working remote and visiting our families and clients.
