Patient Navigator Special Patient Population- Duke University Hospital - Day Shift

Department Profile:
The Children's Health Center and Lenox Baker Children's Hospital at Duke University Hospital is comprised of pediatric outpatient clinics. Our team is active in delivering exemplary care for each patient to ensure an excellent patient experience, every time.
The Complex Care Clinic at Duke Children's is active in delivering exemplary care for each patient to ensure an excellent patient experience.
This position will help to provide support for this high acuity patient population in collaboration with dynamic multidisciplinary teams. This position will work closely with the faculty directors of Pediatric Complex Care Team. We are looking for someone with the ability to establish a strong rapport with patients and families by phone, assist nursing in coordinating services, and reinforce education to patients and families in and out of the clinic setting.
Work Hours & Shifts:
Full Time: Hours may very, but will typically fall within a standard Monday-Friday, 8am-5pm schedule.
Work Performed
- Ensure comprehensive, timely, and seamless care through development and support of care plans, coordination with medical team members, and providing patient education and support
- Exhibit extensive understanding of patient care and health system pathways and serve as a conduit between patient and providers to address medical and psychosocial needs.
- Document all patient communication in Maestro Care (Epic)
- Assist with coordination of multidisciplinary clinics
- Navigate complex systems and processes to maintain patient centered care and facilitate interdisciplinary communication among care team
- Bridge gaps in care and assess/troubleshoot transitions in care and barriers to care.
- Help maintain data bases
- Provide other related duties incidental to the work described herein.
- Education, Bachelor's degree in a clinical field, such as behavioral/social sciences, health education, or public health fields.
- Experience, Three years of experience in a patient-centered, health related field.
- A solid background in a patient-centered health related field
- Preferred experience with patient navigation
- Strong oral /written communication skills, software navigation abilities (EPIC experience preferred)
- Good organizational and time management skills; attention to detail
- Flexible attitude with the ability to function both independently and as a team member
- Ability to solve complex problems with well developed critical thinking skills and communicate effectively with patients, families, providers and leadership
