Safely manages the environment of care. This position will assist our location in a PRN status covering shifts as needed.Essential FunctionsActively reviews patients' drug therapy. When appropriate, consults with physicians to recommend alternative therapy to achieve therapeutic goals.Documents interventions, medication variances, adverse drug reactions, and formulary changes into appropriate data bases.Uses pharmacy resources effectively. Identifies and suggests appropriate alternatives from the formulary.Provides accurate, concise, and effective drug information to physicians and other health professionals.Knowledgeable about the medications, typical dosages, expected effects, side effects, and appropriate reversal agents.Able to interpret clinical laboratory values when applicable to drug therapy.Has the ability to perform pharmaceutical calculations necessary for the preparation of medicationsAble to recognize dosing needs according to patient's weight (actual vs. ideal in the obese patient) for drugs dosed per kg.Understands different sources of drug information and is able to use information resources available for the retrieval of medication related information.Demonstrate the ability to read and interpret medical literature and draw clinical conclusions from review of data.Demonstrate ability to collect and interpret drug therapy data.Understands and is able to perform renal dosing adjustments when applicable.Demonstrate ability to interpret laboratory data when required to monitor drug therapy and makes recommendation to drug therapy based on interpretation of results.Demonstrate the ability to monitor drug therapy response and make clinical recommendations to improve patient care and safety.Understands processes and is able to perform IV to PO conversions.Able to identify basic intravenous compatibility, osmolarity issues related to IV therapy.Able to perform intravenous admixtures following aseptic technique practices.Demonstrate the ability to use the laminar and chemotherapy hoods.Demonstrate the ability to operate basic Pyxis functions (adding, deleting and modifying users, editing and removing meds, view and solve discrepancies, view and print profile overrides).Possesses basic pharmacokinetic skills and demonstrates ability to calculate and recommend initial doses.Behavioral StandardTreats everyone as their customer; utilizes scripting and other tools to ensure consistency in customer service; Expresses recognition and shows appreciation to others; fully utilizes AIDET principles; responds quickly to handle requests, complaints and questions; displays a positive attitude.Communication/KnowledgeWears nametag properly; follows dress code policy; answers phone correctly and promptly; is prepared for meetings; meets deadlines; acts ethically and treats others with respect; respects customer's and co-worker's time; establishes and maintains effective relationships with customers and co-workers.Collaboration/TeamworkAttends staff meetings; follows HIPAA guidelines; follows patient rights policy; complies with the compliance program; demonstrates knowledge of role in a disaster; demonstrates knowledge of fire and fire drill procedures; working knowledge of hospital emergency codes; always utilizes standard precautions in the clinical setting; safely manages the environment of care by demonstrating a working knowledge of the requirements of the: Life Safety program, Utilities Management program, Hazardous Materials program, Emergency Preparedness program, Safety Management program, Medical Equipment Management program, Security Management program. RequirementsExperienceMinimum of 1-2 years of experience in a hospital pharmacy setting preferred.EducationGraduate of a college of Pharmacy with a B.S. degreeLicensure/CertificationCurrent professional registered license with the State Board of Pharmacy
