This position is designed to facilitate an effective process of the Mercy Care Management Model supporting quality patient care, patient safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; collaborating with facilities to include leadership coordination as evidenced by metrics, optimizing performance and adoption of best practice.Sets expectations, develops plans, and manages processes to measure, assess and improve quality, patient outcomes and Care Management metrics.Responsible for development and monitoring key operational aspects such as budget, Care Management metrics, human resource planning and customer service.Collaborates with Care Management and Mercy facility leadership in the development of performance improvement indicators and best practices, identification of clinical and operational opportunities for improvement and is responsible for outcomes.Communicates performance improvement indicators, best practices, clinical and operation opportunities for improvement with Care Management co-workers.Develops and supervises action plans to implement identified improvements to resolve problems, increase effectiveness and efficiency, maximize resources and/or decrease cost and length of stay as evidenced by metric dashboards.Reports the analysis of Care Managements performance, statistical data and results of completion of action plans to Care Management and Mercy facility leadership Provides direction and oversight management for the assigned inpatient Care Management department.Collaborates with Care Management and Mercy facility leadership in developing, evaluating and revising assigned Care Management Department to meet Mercy facility and customer needs.Performs as a resource in the field of care management, discharge planning, social work, utilization management as needed.Oversees the process for delivering of non-coverage letters and other notices to inpatients.Analyzes patient case cost and length of stay reports.Analyzes/reviews reports involving quality and quantity of Care Management co-workers performance and is responsible for providing performance improvement feedback to Care Management co-workers supervised.Directs Care Management activities involved with the accreditation process. Implements Care Management processes to ensure optimal risk management and regulatory compliance.Assesses identified potential risk management or clinical quality issues and resolves them through intervention or collaboration with other members of the healthcare team as appropriate or refers the issue to the appropriate individual(s).Interprets accrediting agency standards of care, State and Federal regulations and/or other review organization standards and third-party payor contracts for Care Management coworkers.Keeps accurate, updated Care Management co-worker files which contain documentation of current licensure, Cardiopulmonary Resuscitation (CPR) certification, record of continuing education and mandatory inservice attendance.Qualifications:Experience: 3 years clinical experience in social work, case management, or related field.Required Education: Bachelors in Social Work (BSW) is required.
