Assess the physical and psychosocial health of individuals and families by obtaining health, medical and social histories using the problem oriented (SOAP) recording system. Initiate, evaluate and modify the effectiveness of therapeutic plans, collaborating appropriately with other members of the health care team. Assess the patient’s level of health education and provides appropriate teaching. Order laboratory and radiographic studies as necessary for the determination of health status according to guidelines. Discriminate between normal and abnormal findings. Evaluate results and determines which findings require consultation or referrals to other Providers. Work with the community to assess health needs. Assist in the development and implementation of programs designed to meet community needs. Maintain accurate records, and maintain referral books and tickler files for follow-up of specific problems. Attend and contribute to clinical meeting of specialty and other meetings. Maintain current awareness of new concepts, methods, techniques, regulations, etc., related to the Nurse Practitioner role Consistently supports the department’s team approach to provision of quality service to its customers. Adjust to new, different, or urgent work situations. Perform other related or similar duties as directed, or as responsibilities necessitate. Maintain current Basic Life Support “C” Certification. Ideal Candidates will have a minimum of 2 years of experience. Must be a graduate of an accredited Nurse practitioner program. Current Massachusetts licensure is required authorizing practice in an expanded role as well as current DEA number and Mass DPH registration to prescribe controlled substances. Knowledge of a second language may be required.
