This position is part of a new initiative in addressing members needs who are most vulnerable.This role requires face-to-face visits within Queens County, NY.Required Qualifications- 2 plus years experience providing clinical services; experience in community/outpatient setting as well as Public HealthPreferred Qualifications- Bi-Lingual (Spanish) - Experience in serving the Medicare Populations in urban or rural environments with familiarity of resources within - Preferred skills: Health Care Condition knowledge like Diabetes, Hypertension, Cardiovascular Disease - Experience using a computer including the use of Excel and Electronic Health Records- Bachelors in Community Health and/ or Social WorkEducationHigh School Diploma or equivalent G.E.D.Pay RangeThe typical pay range for this role is:$18.50 - $35.29This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
