FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

Shift: Days

Pay: $54.31 - $81.47 / hour. Pay is dependent on applicant's relevant experience

Summary:

This position is responsible for the development and implementation of ambulatory clinical pharmacy services and initiatives that optimize the patient care experience and drive organizational performance in the neurology space. The pharmacist specialist in neurology will establish a robust clinical pharmacy practice focusing on patient-centered care and supporting medication management, therapy optimization, and certain pharmacy operational aspects of care delivery related to general and complex medical neurology.

Requirements:

  • Doctor of Pharmacy from an accredited school of Pharmacy.
  • State licensure as a Pharmacist and any relevant life support certification as determined at position level.
  • 5 years specialty experience OR PGY1 or clinical fellowship completion with 3 years' pertinent work experience OR Pertinent PGY2 completion.
Preferred:
  • Board certification in area of expertise within 1 year of employment
Responsibilities:
  • Develops and administers drug therapy protocols as a part of an interdisciplinary team.
  • Identifies high-risk patients and implements evidence-based interventions to optimize medication-related outcomes.
  • Designs and provides patient education and medication management to patients and their families. Mentors pharmacy staff on areas of expertise.
  • In conjunction with a patient's healthcare provider, provides, delegates, and coordinates patient care. Develops and leads quality, safety, and affordability-related initiatives at clinic/unit, service line and department levels.
  • Maintain accountability for utilization of UCHealth outpatient pharmacies for neurology medications, focusing on medications requiring specialty pharmacy services.
  • Documents interventions and their outcomes for use in drug therapy quality improvement efforts. Participates in research and committees for the purpose of evaluating and advancing standards of care.
