****Job Description?****

Under the oversight of the clinical dietitians and in collaboration with other members of the health care team, identifies patients at nutritional risk through the nutrition screening process and the evaluation of nutritional risk factors. Assesses nutritional needs and defines nutrient requirements for patients at moderate nutritional risk, develops and implements nutrition care plans, and evaluates the effectiveness of the nutrition interventions. Provides basic nutrition education to patients on cardiac, low sodium, and other designated diets and on infant feeding guidelines, and refers patients to the dietitians as appropriate in-depth counseling. Promotes continuity of care through effective discharge planning, communication with other care providers, and post-discharge phone follow-up with patients. Assists with diet clerk coverage as needed
