This person must report into our Boston, MA office 3 days/week. Where you’ll see your impact: The Care Coordinator will be the first person to meet with the patient after CCM/PCM/RMP is ordered. Proactively identify and work with people, including the elderly and those with long-term cardiopulmonary conditions, to provide coordination and navigation of care and support across health and care services. As a Care Coordinator, you would do an overview of the patient's care and care plan/update care plan. Make sure the patient has the appropriate meds and knows how to use them, make sure they have the appropriate DME and knows how to use them as well. Review ER and hospitalization history, etc. What you’ll be responsible for: Accountable for helping patients keep up with their care plans. Coach patients to reach their care goals by following treatment plans that include healthy eating, physical activity, stress reduction, etc. Develop personalized care plans to address patients’ physical, mental, and preventative health needs. Manage and support patients’ healthcare needs via phone conversations. Conduct and document visits by using several technology platforms and EHRs. Connect patients with resources such as nurses, respiratory therapists, and doctors who can also assist them in meeting their care needs. You would be a good fit: Healthcare experience as a background in the medical field is a must have. Must have experience working with patients with pulmonary diseases.
