The onsite Community Health Worker will support the Medical Team to help coordinate vaccine clinics, educate patients about COVID regarding testing and treatments. Position Summary: The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services as part of the population health team. The Community Health Worker will support the COVID Medical Team. Essential Duties and Responsibilities: Maintains working knowledge of and adhere to the program Manual, agency handbook and Policies and Procedures Commitment to providing cost conscious, fiscally responsible, timely, efficient, effective, safe, patient-centered, culturally competent, equitable, and participatory health care. Attends all required trainings as per Agency Policy, Program Manual and Funder Guidelines. Arrives on time for work and remains at work for whole shift and have regular and predicable attendance. Maintains appropriate boundaries and ethical standards with staff, clients, and visitors and outside providers as detailed in Agency Policy. Maintains positive and professional working relationship with co-workers, managers, clients, visitors and outside providers Strong and clear written and verbal communication skills Excellent computer skills (included but not limited to Microsoft Word, Excel and Outlook) Embodies the following values as set forth in the strategic plan: Compassion, Knowledge is power, equity and inclusion, excellence, courage and capacity to change. Any and all other duties as assigned. Program Specific Essential Duties and Responsibilities Supports the care team with patient follow-up, reminders, and addresses barriers to care. Document all services as required and will assist in meeting all program requirements. Collects and analyzes data related to the project and works with data management staff for data collection. Provides outreach and meets with patients to explain services and coordinates the treatment plan. Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable. Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services. Strong organizational and communication skills to act as a hub of information across multiple departments. Any and all other duties as assigned. Qualifications and Education Requirements One of the following combinations of education and employment experience must be met in order to be considered for the position: Education And Experience High school diploma or GED required; Bachelor's degree is preferred. Preferred candidate will have Community Health Worker or Medical Interpreter certificate of completion. Minimum of two years relevant work or college-level internship experience required. Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners. Bilingual in English and another language appropriate to FHCW patient population required. Community Health Worker Core Training required within first year of employment.
