We are looking for a Care Navigator to play an important role at AndHealth. In this role, you'll help guide the patients through their journey at our customer location in Zanesville, OH with the goal of creating incredible health outcomes and ensuring they receive the care needed to live a healthier life.
You'll work with our Physicians, Nurses, Health Coaches, and Patient Services team and provide feedback on how to best deliver a "wow" experience.
What you'll do in the role:
- Properly prepare exam rooms for patient care by ensuring the room is properly stocked and cleaned.
- Monitor patient phone calls, receives, and routes test results for provider review, and collects data regarding patient medications.
- Room patients, check vital signs, and assess patient needs to communicate with provider.
- Assist provider with patient procedures as directed by the provider.
- Communicate effectively both written and orally with patients, patients' families, coworkers, and other outside entities.
- Complete routine forms for patients as appropriate.
- Ensure chart documentation in the Electronic Health Record (EHR) is correctly completed in a timely manner.
- Perform lab tests as directed by provider, prepares specimens, operates lab equipment, and maintains laboratory documentation in accordance with CLIA standards and practice specific protocols.
- Reconcile billing from offsite labs by comparing daily lab log with patient EHR.
- Schedule offsite diagnostic tests and communicate effectively with patients.
- Facilitate referral processes for patients as needed.
- Take manual inventory of drugs.
- Perform front office duties as needed.
- Adhere to all community health center policies and procedures.
- Other responsibilities as needed based on business needs.
- Bachelor's degree is preferred.
- LPN Preferred.
- RN Preferred.
- American Heart Association Basic Life Support Provider (BLS) obtained or willing to obtain.
- Experience working in an ambulatory or outpatient setting preferred.
- Proficiency in Microsoft Office.
- Ability to navigate Electronic Medical and Health Record (EMR, EHR) systems.
- A keen sense of ownership and ability to take initiative to make each patient's experience better than the last.
- Excellent written and oral communication skills.
- Strong attention to detail and ability to successfully handle multiple things at once.
- You bring a servant-leadership approach to your interaction with everyone.
- You are comfortable with technology and learning new systems.
- You thrive when collaborating with others from multiple organizations.
- (PPD) TB Skin Test - Proof ofnegativeTB skin test within thelast 12 months.
- (MMR) Measles, Mumps and Rubella or A Blood Titer provingimmunity.
- Varicella -(2)documented doses or A Blood Titer proving.
- HepB3 Series.
- (Flu) Influenza required.
- COVID Vaccine required.
