Reports to:Pharmacy Manager and/or Nurse Manager of Cancer Services & AIC

GENERAL SUMMARY

Works under the supervision of the Pharmacy Manager and/or Nurse Manager of Cancer Services & AIC. Day-to-day Cancer Center Pharmacist duties are supervised by the Registered Pharmacist Oncology Lead. The primary job function of the Registered Pharmacist Oncology is to distribute medications and supplies according to departmental procedures with the assistance of pharmacy technicians. Collaborates with physicians and other hospital staff on medication related matters. Educates patients and their families to optimize drug therapy.

QUALIFICATIONS

Education:

  • High School Diploma or successful completion of an equivalent High School Exam Required
  • Bachelor's degree in Pharmacy (5 year) is required
  • Doctor of Pharmacy (Pharm D) is also preferred
Licensure:
  • Pharmacy(OH) license required
  • Graduate Pharmacists may also be hired awaiting license
  • BLS within 90 days of hire required
Experience:
  • Previous hospital experience preferred
Interpersonal Skills:
  • Must be able to communicate effectively with hospital, physicians, and nursing personnel when clarifying medication orders and dispensing pharmaceutical supplies.
  • Must be able to provide drug information to a variety of clinical personnel.
  • Must assign work and provide guidance to departmental personnel.
Essential Technical/Motor Skills:
  • Inputting data/typing, speaking clearly, answering telephones, precise eye/hand/foot coordination, fingering/fine dexterity, delicate/precise manipulation, handling/gripping.
Essential Physical Requirements:
  • Lifting/transferring/carrying light (20 lbs. Maximum).
  • Pushing, pulling, squatting, kneeling, reaching, positioning, standing, holding, walking repetitive activities.
Essential Mental Requirements:
  • Calculations, manipulating numbers, interpreting numbers, analyzing, forecasting, assessing/evaluating, explaining/teaching, synthesizing, attention to detail, spatial/form perception, memory, problem-solving/reasoning.
Essential Sensory Requirements:
  • Ability to see, hear, feel, taste/smell, distinguish color.
Exposure to Hazards:
  • Various chemicals, electrical apparatus, mechanical apparatus.
  • Eye strain from detailed work and computer operation.
  • OSHA category I - tasks that involve exposure to blood, body fluids or tissues.

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS

  1. Checks the accuracy and appropriateness of all medications dispensed.
  2. Intervenes clinically with the provider when necessary and appropriate to optimize drug therapy.
  3. Maintains accurate and appropriate patient profiling and charging records for all inpatient and non-inpatient medication orders.
  4. Follows accepted standards of practice and adheres to federal, state, and local regulations, as well as the policies and procedures of SOMC Cancer Services and Pharmacy Services.
  5. Serves as a drug information source for patients, physicians, nursing personnel, hospital staff, visitors and the community.
  6. Coordinates with the Registered Pharmacist Oncology Lead for ordering, storage, and handling of medications including chemotherapy, hazardous, and non-hazardous drugs and supplies.
  7. Compounds and dispenses medication and pharmaceutical supplies according to accepted standards of practice, including adherence to safety requirements in the handling and preparation of chemotherapy, hazardous, and non-hazardous agents according to current guidelines (USP 797, USP 800, NIOSH) to fill prescriptions issued by qualified prescribers.
  8. Intervenes clinically by assisting with clinical monitoring by laboratory assessment of appropriate CBC, renal, hepatic, electrolyte, and other pertinent measurements and patient specific criteria as recommend for safe delivery of chemotherapy, hazardous, and non-hazardous agents when necessary and appropriate to optimize drug therapy.
  9. Participates in identification and reporting of therapy errors, adverse drug reactions, or other related near miss opportunities related to safe medication supply and delivery.
  10. Serves as medication information source for patients, physicians, practitioners, nursing personnel, hospital, and clinical staff, visitors, and the community.
  11. May educate patients and their families to ensure the understanding of the goal, administration schedule, toxicity management of chemotherapy, immunotherapy, targeted therapy, and hematology treatments, and documents the activity and interventions in the EMR.
  12. Educates providers related to toxicity management of chemotherapy, immunotherapy, targeted therapy, and hematology treatments as applicable.
  13. Maintains a level of professional competency and remains current in the field of pharmacy.
  14. Performs other duties as assigned.
