Our Ideal Candidate: We are looking for a team member who is committed to providing overseeing health care supplies fora all sites of the organization to assure safe and efficient delivery of care to patients by YNHS personnel including through staff outreach efforts. Some technical requirements for this position are:
  • Five years of management level experience, including at least 3 years' experience in health or social services.
  • Maintaining current expertise in the areas of safety management and emergency preparedness.
  • Assist with Environment of Care (EOC) coordination at all facilities and mobile units as well as assisting with safety plans, policies, and risk assessments are current and reflect the requirements of accrediting and regulatory agencies.
  • Identify, evaluate,and communicate risks in the EOC to leadership, works with Quality and Compliance teams to implement procedures that achieve the lowest possible risk and adverse impact to patients and staff.
  • Candidate has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency.
Day to Day:
  • Oversees medical supply staff and acquisition process to maintain adequate inventory for provision of services at all locations.
  • Monitors placement of equipment and supplies throughout YNHS facilities to assure safe access for patients, staff, residents, and visitors throughout facilities and YNHS owned properties.
  • Assists in development and provision of safety training for employees on policies, regulations, and procedures (security, fire extinguishers, emergency exits, etc.)
  • Monitors the removal of biological, physical, and chemical hazards from the workplace.
  • Work with Facilities team and Safety Committee to monitor site safety checks, analyze results, report results to Quality team designee.
  • Work closely with quality team designee to maintain accurate and current monitoring logs of facility maintenance, safety monitoring and other reporting as outlined in YNHS policy manuals (EOC, safety, housing and maintenance).
  • Works with department supervisors to assure compliance with regulations governing the organization.
  • Train, supervise, and oversee Care Coordinators in all YNHS sites in principles of care coordination. Serve as liaison with state contracting agencies, community partners to coordinate resources and referral patterns.
  • Monitor care coordinators in activities (coordination, transitions, referral management, health promotion, individual and caregiver support) according to program regulations, documentation in YNHS E.H.R. as well as external registries and forms.
  • Participate in quality assurance activities and peer review as requested.
  • Maintain organized, clean, efficient, and confidential work area.
  • Represent YNHS in local meetings as requested.
  • Perform related work or other tasks as requested.
