Job Description Responsible for daily staffing of scheduled patient visits and PRN visits as directed by patient care needs and acuity. Responsible for ensuring that skill level needed by patient matches the skill level of the nurse or therapist assigned. Works collaboratively with central intake, contracted therapy, agency scheduling offices, case managers, supervisors and field staff to assure scheduled visits are completed timely. Notifies other disciplines of start of care date and of order for service. Assigns patient to appropriate team and Case Managers or therapist. Maintains schedules in scheduler program. Ensures that paperwork turned in is complete. Contacts patients prior to admission visit to facilitate admission process. Can assist with triage as needed.

Job Requirements **REQUIRED QUALIFICATIONS**

**Education:** Must have high school diploma.

**Skills:** Must be able to understand directions, communicate, and respond to inquiries; requires effective interpersonal communication skills. Must have excellent customer service skills, organizational skills and problem solving abilities. Must be a team player, flexible, and able to handle multiple tasks. Willing and able to learn computer skills. Must be a team player, flexible, and able to handle multiple tasks.

**Years of Experience**: Must have recent clinical experience and/or minimum of 2-3 years of homecare experience.

Must be able to work weekends.

**PREFERRED QUALIFICATIONS**

**Years of Experience:** Staffing experience preferred.
