Bright Harbor Healthcare provides PACT services to more than 160 people in all of Ocean County. The PACT program is a multi-disciplinary team for persons with a serious and persistent mental illness. A team consists of a psychiatrist, advanced practice nurse, nurses, clinicians, co-occurring specialists, vocational specialists, mental health specialists, and peer mental health specialists.

Salary:

Responsibilities:

  • Participate in multi-disciplinary treatment team for collaboration of client care
  • Provide case management services including consumer advocacy
  • Assists consumers with obtaining basic needs and activities of daily living through modeling, direct assistance with ADLs and coaching
  • Provides on-going assessment of client's emotional well being, clinical symptomology, behavioral changes and medication compliance
  • Provides psychosocial education for consumers and family members on mental illness, symptom management, medication compliance and life skills
Requirements
  • Graduation from an accredited school of nursing
  • One year experience working with consumers with mental illness
  • Current license to practice as an LPN in New Jersey
  • Valid NJ drivers license with less than 6 points
