ESSENTIAL JOB FUNCTIONS:
Comply with all applicable rules, policies, standards, and guidelines related to employment with Westminster Services and its communities.
- Work as a community liaison to develop and maintain relations with physicians and hospitals on an ongoing basis, insurance-based case managers, or marketing personnel on all sub-acute patients.including for example:providing them with Medicare updates and changes in facility procedures.
- Ensure that the budgeted census does not decrease below the established minimum.
- Periodically assess services provided by the facility; compare these to the needs of the community. Assess how the current services compare to those offered by other comparable facilities in the area.
- Assist in the evaluation of all potential residents to ensure appropriate placement.
- Ensure proper federal, state and facility mandated documentation is completed. Ensure all necessary Medicare and/or insurance information if available and confirmed prior to admission.
- Works with residents, families and other staff as needed to ensure that residents are evaluated for the appropriate level of care.
- Educate residents and families about the extent of Medicare and Medicaid coverage, including length of stay, amount of coverage, and co-insurance days.
- Address quality measures issues for potential admissions and returning residents.
ESSENTIAL QUALIFICATIONS:
Education/Licensure:Prefer a licensed RN or LPN in the state of Florida; or possess a degree in a related healthcare field.Must be CPR certified.
Experience:Healthcare Business Development experience is a plus.Two years (three years, if LPN) in long term care and assisted living, including working knowledge of case management/ clinical work, Medicare /Managed care and Managed Medicaid protocols. One year in post-acute rehabilitation. Case management purpose and process knowledge required. Demonstrated clinical assessment skills required.
Physical and Environmental Requirements: Grasping, Kneeling, Lifting up to 40 lbs, Pushing, Reaching, Stooping, and Driving.
