Director of Case Management
and play a pivotal role in providing top-notch patient care and ensuring seamless transitions across our 300-bed acute care facility.
Responsibilities:
As the Director of Case Management, you will lead and oversee our discharge team to ensure efficient and effective patient transitions and care coordination. Your responsibilities will include:
Providing strategic direction and leadership to the case management department.
Collaborating with interdisciplinary teams to develop and implement patient-centered care plans.
Managing utilization review processes and ensuring compliance with regulatory standards.
Identifying and implementing process improvements to enhance patient care and outcomes.
Mentoring and developing case management staff to achieve excellence in their roles.
Utilizing data and analytics to drive performance improvements and cost-effective care.
Requirements:
Bachelor's degree and graduate of a Bachelor's Program in Nursing.
Professional Registered Nursing licensure in the state of Georgia (or compact license).
Minimum of five (5) years of experience as a Director of Case Management in a short-term acute care hospital setting.
Proven leadership skills and ability to manage and develop teams.
Strong understanding of utilization review, care coordination, and discharge planning.
Excellent communication, interpersonal, and organizational skills.
