Our robust training program allows staff members to develop and grow in the position and provides the basis for upward mobility within the agency.

The Intake Specialist supports the mission of Cornerstone Montgomery by coordinating the referral and intake process of all assigned programs ensuring a steady pool of appropriate candidates and increasing program census.

*Responsibilities of the Intake Specialist*
* Provide outreach services and engage clients within appropriate timeframe of being referred to program services.
* Determine if clients meet eligibility requirements for program prior to intake. Refer people who request services not within our purview to the appropriate outside agencies
* Provide planning, coordination and linkage of services for all intake clients, including completing psychosocial and diagnostic assessments of clients referred to determine appropriateness for services, both on and off site, and documenting services in electronic health record.
* Assign clients to appropriate program after intake and distribute intake assessments to appropriate program for referral. Coordinate and communicate with other program staff, the Local Behavioral Health Authority, outside referring agents/professionals and families.
* Manage internal waiting list for agency services and respond to phone calls on the agency intake line; request authorizations; submit new client information to DORS, as applicable.
* Participate in agency case planning meetings, program development activities, policy and procedure development, and training
* Performs other duties as assigned

*Qualifications of the Intake Specialist*
* BA in Social Work or related field. Master’s preferred.
* Minimum two years experience working with serious mentally ill population in the capacity of case manager or counselor.
* The use of a personal vehicle as needed is required (mileage reimbursed 62.2 cents per mile)
