Consider joining a growing team of Service Coordinators - HUD with Lutheran Social Services of WI & Upper Michigan. Lutheran Social Services of WI and Upper MI is currently seeking a HUD Service Coordinator to join our team. This position will provide community resources to tenants with disabilities in a HUD building in order to help them maintain their housing. The position will cover a property in Butler, WI. This is a 40 hour per week position. M-F, Flexible Hours. Under the direction of the Program Supervisor, the Service Coordinator HUD provides information and referrals to community connections to tenants following guidelines, as defined in the HUD Housing Handbook Section 202, who need supportive community services to maintain self-sufficiency. ESSENTIAL DUTIES AND RESPONSIBILITIES: This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time. Provide general informal case management, including intake, and referral services to all tenants needing such assistance when case management is not available. Assessment of health, psychological and social needs. Development of an individually tailored case plan for services and periodic reassessment of the tenant's situation. Establish linkages with all agencies and service providers in the community. Sets up a directory of providers for use by both project staff and tenants. Refers and links the tenants of the project to service providers in the general community. Educates tenants on service availability, application procedures, client rights, etc. May develop case plans in coordination with assessment services in community. Monitors the ongoing provision of services from community agencies. May set up volunteer support programs with service organizations in the community. Helps other tenants build informal support networks with other residents, family and friends. May provide training to project tenants in the obligation of tenancy or coordinate such training. May educate other staff on the management team on issues relating to aging in place and service coordination. Presents workshops or informative sessions to tenants on subjects of tenant interest. Demonstrate the ability to work with tenants, the agency and the community in a positive manner as well as the ability to work independently. Performs other duties as assigned. EDUCATION AND/OR EXPERIENCE: Bachelor's degree in social work, sociology, psychology or related field or an equivalent combination of education and/or experience is required with at least 3 years of related experience, including experience working with the older adult population. Working knowledge of supportive services and other resources for older adults and/or persons with disabilities is preferred. CERTIFICATES, LICENSES, REGISTRATIONS: Must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
