Job DescriptionJob SummaryThe Pharmacy Clinical Manager is responsible for designing, implementing and managing the pharmacy clinical programs. This includes formulary management, policy and procedure development and education across the healthcare spectrum. The Clinical Manager shall also assist in managing the pharmacy when the Pharmacy Director or designee is unavailable.Major Responsibilities:Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer systemAccurately compounds and dispenses pharmaceuticalsComplies with facility policy and procedures and regulatory requirementsMonitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and doseProvides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as neededSustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selectionSupports participation in daily multidisciplinary and bedside rounds (may need to attend)Identifies, designs and analyzes drug utilization reviews to promote rational drug therapy and determines action plan based on resultsProvides accurate and timely medication information to healthcare staff and patientsDevelops, manages and mentors pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelinesDevelops, manages and mentors for the antimicrobial management program (AMP)QualificationsB.S. or Pharm D. from an ACPE-accredited School of Pharmacy - RequiredDoctor of Pharmacy from an ACPE-accredited college of pharmacy - PreferredMinimum of 1 year as a Pharmacy Clinical/Operations Specialist - RequiredMin 5 years of hospital pharmacy experience required; preferably within area of focus- RequiredCompletion of hospital PGY2/ pharmacy specialty residency or fellowship - PreferredSpecialty BCPS certification - PreferredOther certification (e.g. FCCP, FASHP) Preferred •Hospital Pharmacy Management experience - PreferredExperience performing basic mathematical calculations, balance and reconciliation of figures - RequiredExperience working in Microsoft Office, Meditech, Business Objects and purchasing vendor applications - RequiredLicenses, Certifications, & Training:ACLS certification PreferredActive Pharmacist license in good standing with the appropriate State Board of Pharmacy - RequiredActive Pharmacy Consultant license, or equivalent as per State requirements for a Pharmacy - RequiredWhy is This a Great OpportunityGreat benefitsLocations San Antonio
