Palmetto Primary Care Physicians (PPCP) is the largest independent multi-specialty group practice in South Carolina and has proudly served patients and our communities since 1996. The group is comprised of over 100 clinical providers with a wide range of expertise in primary and specialty care including endocrinology, gastroenterology, neurology, psychiatry, audiology, and physical therapy. PPCP offers a wide range of services, including a state-of-the-art urgent care clinic, imaging, diagnostics, and labs. The company employs over 500 clinical and non-clinical staff throughout South Carolina and provides exceptional medical care for patients in over 40 offices.

Job Functions:
  • Triages patients within the multi-disciplinary team based upon identified goals.
  • Performs transitional care management services.
  • Performs comprehensive patient assessments that include: comprehension of injury, illness, or condition, medication reconciliation, need for follow-up visit to PCP, need for Care or Treatment plan.
  • Performs post-visit coaching that includes: follow up to ensure understanding and compliance of instructions.
  • Instructs patients telephonically in disease management care.
  • Creates patient specific care plans to promote improvement of health outcomes and compliance.
  • Assists patients with appointment scheduling for follow-ups, identification of community resources, and other resources (Home Health) as needed.
  • Other duties as assigned
Requirements
  • Education: Bachelor - RN required
  • Experience: Minimum of 5 years' experience in a clinical setting, including utilization management, case management or discharge planning;
  • Licensure/Certification: Current RN licensure (South Carolina)
