SUMMARY

The Transition Coordinator (TC) Team Lead functions as the resource to other Transition Coordinator team members with the responsibility of working with clients, families, providers and other staff. The TC Team Lead provides support to the Manager, Clinical Operations with hiring, training and retention of Transition Coordinators. Performs all duties in a manner that fosters the achievement of the organization's mission to identify choices and provide services to help people of all ages, abilities and incomes to live at home.

KEY RESPONSIBILITIES
  1. Coordinates, manages and implements internal tracking tools and workflows to ensure quality and accuracy.
  2. Serves as a mentor and resource to Transition Coordinators to include ongoing training, case reviews, and shadowing/training of newly hired Transition Coordinators.
  3. Tracks, records and reports on Transition Coordinator related successes, failures, and areas for improvement.
  4. Works with manager to monitor individual progress and assist in skill development or training when necessary.
  5. Performs Transition Coordinator tasks including transitioning consumers, meeting with consumers, their families, and facility social workers, gathering identifications, and assisting with applications for state and federal programs.
  6. Monitors, tracks and documents process improvement strategies.
  7. Maintains, updates and continually develops Transition Coordination training manuals.
  8. Attends and/or facilitates meetings and trainings as required.
  9. Performs other related duties as assigned.

Requirements

QUALIFICATIONS

Education
  • High School Diploma or equivalent; Bachelor's Degree in Marketing, Human Services or related field preferred.
Experience
  • Demonstrated success in Transition Coordinator role at CCC.
  • Knowledge of community and housing resources in Connecticut, with the ability to understand and articulate multiple programs.
  • Strong communication skills, both written and verbal.
  • Ability to keep organized and work effectively in a fast-paced, dynamic environment.
  • Proficiency with computer programs, especially Microsoft Office suite.
  • Knowledge of the Independent Living philosophy is helpful.
  • Familiarity with the needs of a geriatric population is helpful.
  • Familiarity with general office procedures, equipment, and telephones.

Work Environment (a brief description)

Work is performed primarily in an office environment but may also include visiting client in hospitals, nursing homes and other locations.
