Responsibilities

Maintain accurate and up-to-date medical records, documenting patient assessments, care interventions, medications administered, and other relevant information

Participate in quality improvement initiatives aimed at enhancing the delivery of care, improving patient outcomes, and ensuring compliance with quality standards

Complete medical necessity and level of care reviews for requested services using clinical judgment and refer to Medical Directors for review

Provide clinical knowledge and act as a clinical resource to non-clinical team staff

Collaborate with various staff within provider networks and discharge planning team to coordinate member care

Required Qualifications

• 1+ years case worker experience - Working with the adult, blind and disabled community over the age of 21

• Strong computer skills - MS office

• LVN, LPN, or LCSW required

• Bilingual preferred
