Overview: The Licensed Vocational Nurse (LVN) plays a crucial role in delivering high-quality patient care under the guidance of a registered nurse. This position adheres to the established care plan, relevant regulations, and accepted nursing standards while ensuring compliance with Interim HealthCare policies.

Education & Experience Requirements:

  • Must be a graduate from an accredited vocational nursing program and possess a valid license to practice in the state of employment, with the license in good standing.
  • Preferred experience includes a minimum of 1200 hours working as an LPN within the past year or participation in a preceptor program.

Key Responsibilities:

  • Monitor and report any changes in the patient's health status to the supervising RN.
  • Document clinical notes accurately and promptly, reflecting the patient's progress.
  • Assist the RN or physician with specialized procedures and tasks.
  • Support the RN in implementing the patient's care plan.
  • Guide clients in learning effective self-care practices.
  • Capable of performing CPR when necessary.
  • Adhere to established nursing practices and standards.
  • Willingness to travel within the designated service area for assignments.
  • Complete additional tasks as assigned.
  • May have access to protected health information (PHI) as required for job duties, with access and disclosure governed by organizational policies.

Skills & Abilities:

  • Ability to manage and prioritize multiple tasks effectively.
  • Proficient in reading and interpreting technical instructions related to patient care.
  • Skilled in observing patients both visually and auditorily.
  • Adaptable to changing patient and scheduling needs.
  • Must provide proof of a valid driver's license if applicable.
  • Must provide proof of valid auto liability insurance if driving is required for patient transport.
  • Effective communication skills in English, both written and verbal, with all levels of staff.
  • Ability to report clinical information clearly to clients, caregivers, and agency personnel.
  • Two positive professional references are required.
  • Must meet applicable health screening requirements.
  • Pass federal and state criminal background checks as required.
  • Verification of employment eligibility (I-9) in accordance with local, state, and federal regulations.

Work Environment & Physical Demands:

  • Work is typically conducted in patients' homes, educational institutions, or workplace settings.
  • Frequent exposure to communicable diseases and hazardous materials necessitates adherence to safety protocols and the use of protective equipment.
  • Physical activities include standing, walking, bending, stooping, squatting, kneeling, and assisting patients with mobility.
  • Requires manual dexterity for tasks such as using syringes, vials, and other medical instruments.
  • Medium physical activity level, with the potential for lifting, pushing, or pulling up to 50 lbs. as part of daily responsibilities.
