Responsibilities

Coordinate access to health-related services by assisting clients with utilizing available resources.

  • Identify potential clients, complete initial need assessment, and maintain follow-up to ensure appropriate guidance to health services.
  • Provide informal counseling, education, and social support to clients.
  • Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
  • Develop and maintain strong working relationship with external (referral sources and community agencies) and internal contacts (departmental team members).
  • Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.
Requirements

Licensure / Certification / Registration:

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Drivers License preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's
    degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences
  • The ideal candidate will have experience building trust and rapport with patients and demonstrate comfort handling individuals with complex medical or social needs.
  • They should be capable of working independently and making informed decisions with minimal supervision.
  • Proven ability to collaborate effectively with a multidisciplinary team is essential, as well as strong verbal and written communication skills.
  • The candidate should also be adept at resolving issues and finding solutions, and have experience in providing culturally sensitive care.
