Care Coordinator

Care Coordinator reports to the department area manager and provides discharge planning services, psychosocial evaluation, counseling and support to clients admitted to the Crisis program area. The individual works in collaboration with other department area staff to provide a full array of Behavioral Health support.

Schedule
Friday/Saturday 7a-7p

Qualifications

  • Bachelor's degree in human services field from an accredited educational program
  • 2+ years working with individuals with chronic mental illness
  • Ability to work cooperatively with the multi-disciplinary team
  • Excellent written and verbal communication skills
  • Ability to verbally and physically de-escalate a client utilizing BMC trained programs
