Areas of Hospital Pharmacy Practice: Emergency Department, Internal Medicine, Cardiology, Intensive Care Unit, Neonatal Intensive Care, Maternity, Inpatient Central Pharmacy Operations, Oncology & Infusion Centers Responsibilities Job DescriptionJob Expectations:Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.Provide clinical pharmacy services to all patients in assigned area using Standard Operating Procedures (SOP). This includes dispensing of medication, completing review for all patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives as appropriate.Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars, and conferences, and actively participating in department clinical meetings and activities.Participate in regulatory, quality assurance and process improvement activities as required. Meets all regulatory requirements.Uses available information technology and other resources to support day-to-day work operationsOther duties as assigned.Organization Expectations, as applicable: Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served Partners with patient care giver in care/decision making.Communicates in a respective manner.Ensures a safe, secure environment.Individualizes plan of care to meet patient needs.Modifies clinical interventions based on population served.Provides patient education based on as assessment of learning needs of patient/care giver.Fulfills all organizational requirementsCompletes all required learning relevant to the roleComplies with all relevant laws, regulations, and policiesPerforms other duties as assigned.
