JOB PURPOSE OR MISSION: Performs professional social work to facilitate the movement of patients through the continuum of care while ensuring their biopsychosocial needs are met as appropriate for the age population served, as defined in the department's scope of service.
PERFORMANCE CRITERIA

JOB FUNCTIONS
ESSENTIAL JOB FUNCTIONS include, but are not limited to:
1. Provides assessment and subsequent interventions to patients and their families.
PERFORMANCE STANDARDS:
•Assesses patients within 24 hours of notification and completes assessment within departmental timeframes.
•Monitors and assesses issues referring to patients and provides short-term counseling to patients and family members.
•Effectively participates as an interdisciplinary team member in developing the plan of care as appropriate to the assigned area.
2. Facilitates the movement of patients through the continuum of care by the discharge planning process.
PERFORMANCE STANDARDS:
•Initiates discharge plans, communicates and collaborates with other healthcare professionals, and updates discharge plans following department policy guidelines.
•Coordinates community resources and referral sources to achieve optimum results.
•Conducts group sessions, family and individual as per program schedule or indicated.
•Attends and participates in treatment team staffing.
3. Upholds standards of social work ethics prescribed by the NASW and General Health System.
PERFORMANCE STANDARDS:
•Maintains state licensure and/or certification, appropriately utilizes Ethics Committee, upholds professional confidentiality, meets annual CEU requirements.
4. Provides supervision of undergraduates, MSW and LPC's as assigned by supervisor.
PERFORMANCE STANDARDS:
•Sign off on undergraduate assessments as assigned by supervisor.
5. Performs all other duties as assigned.
Requirements
EXPERIENCE REQUIREMENTS
2 years experience in healthcare Social Work preferred.
SPECIAL SKILL, LICENSE AND KNOWLEDGE REQURIEMENTS
Current Louisiana licensure as a Licensed Clinical Social Worker (LCSW).
