Acts as the primary contact for all CFT participants, to facilitate meetings and follow up on recommendations, to ensure continuity of care and service delivery, and educate members about the CFT process. The School Based Outreach Worker provides onsite intakes and services, either on campus or via zoom. This position is the main contact for school districts with established Memorandums of Understanding (MOU). This position coordinates school-based referrals and provides timely responsiveness to those referrals. They ensure the provision of school-based services in the community. This position will work in partnership with different school campuses, providing intakes and groups at the campus. It is a mobile position with a lot of community involvement.
