Job DetailsLevelEntryJob LocationThrive - Tucson, AZPosition TypeFull TimeJob CategoryHealth CareDescriptionWe are looking for a full time School Based Outreach Worker to join the Youth and Family Services Team Responsible for clinical oversight of member care, and to the team as it relates to the delivery of services, including the Child and Family Team (CFT) treatment planning. Acts as the primary contact for all CFT participants, to facilitate meetings and follow up on recommendations, to ensure continuity of care and service delivery, and educate members about the CFT process. The School Based Outreach Worker provides onsite intakes and services, either on campus or via zoom. This position is the main contact for school districts with established Memorandums of Understanding (MOU). This position coordinates school-based referrals and provides timely responsiveness to those referrals. They ensure the provision of school-based services in the community. This position will work in partnership with different school campuses, providing intakes and groups at the campus. It is a mobile position with a lot of community involvement.
