Green Acres Nursing and Rehab (formerly SagePoint Senior Living), located in La Plata, MD is seeking an experienced MDS Coordinator to join our healthcare team. The ideal candidate will have a strong background in patient care and be knowledgeable in areas such as Senior Care.
Qualifications:
-RN License in MD or compact state - Previous experience working in a Long Term Care / Skilled Nursing Facility - Familiarity with ICD-10 coding and MDS assessment process - Experience in senior care - Ability to communicate effectively with doctors and other healthcare professionals
Responsibilities: - Coordinate the completion of the Minimum Data Set (MDS) assessments for all residents - Ensure accuracy and timeliness of MDS submissions - Collaborate with healthcare providers to develop individualized care plans - Monitor and update resident medical records - Conduct assessments to determine resident eligibility for Medicare and Medicaid reimbursement - Provide guidance to staff on proper documentation procedures
