Supported Independent Living Coordinator

We offer a range of support services for Mainers with disabilities and are super excited to expand our remote and virtual support programs. That's where you come in As Coordinator for Supported Independent Living, you'll be part of a team to operationalize, grow, and oversee this new and innovative program. Combining in-person and remote staff with cutting-edge technology, this program will help people with intellectual and developmental disabilities achieve independent living
Key Responsibilities:
Conduct outreach to find and attract clients for this new program which uses a combination of in-person, remote staff, remote monitoring, and enabling technology.
Collaborate with case managers to coordinate services for independent living, including housing, MaineCare authorizations, person-centered planning, consents, etc.
Collaborate with partners (community providers, DHHS) to develop and promote the program within current and emerging care models.
Create and manage individualized care plans tailored to client needs.
Ensure the functionality and efficiency of technology used in remote support to ensure uninterrupted service.
Provide direct care both in-person and virtually, as needed.
Research and implement new technological solutions to enhance our services.
Qualifications:
Bachelor’s degree in social work, Human Services, or a related field.
Experience in a similar role working with Knowledge of IDD system of MaineCare Services.
Strong technological skills and the ability to troubleshoot and manage technology.
Self-starter and skilled in new program development.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Strong organizational and multitasking abilities.
Passion for supporting individuals in achieving a full life.
Willing to travel within the State.
Flexible availability to meet client and program needs.
