Care Coordinator Supervisor Lucas CountyUnison Health is seeking a full-time Care Coordinator Supervisor to lead a team of individuals in the Ohio Rise Program. This position is responsible for promoting the recovery of children/families participating in the Ohio Rise program through the supervision of direct service care coordinators in the following counties: Lucas.Unison Health is a leading resource for people seeking access to behavioral health, substance abuse treatment and primary healthcare services in northwest Ohio. We are seeking passionate individuals to join us in our mission of "creating hope through superior integrated community health care". Education/Experience/Other Requirements:Bachelor's degree in social work, psychology or related field with a master's degree preferred.LSW, LPC, LISW or LPCC preferred.Minimum three years' experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers. Must be proficient in the use of computers and software. Possess a valid driver's license, reliable transportation, be able to be insured through the agency commercial policy and have personal auto insurance. Prior supervisory experience preferred.Duties:Implement the required model of care, provide individualized care based on client need, and link clients to needed services and supports.
