Job Description:

The Care Coordinator manages inquiries for proton therapy from a variety of sources. This includes answering and responding to inquiries from referring clinical providers, patients, caretakers, and others. The Care Coordinator will work with the Radiation Oncologists to determine clinical eligibility for proton therapy for potential patients. The Care Coordinator will then gather clinical data, review, and discern next steps, and assist in the scheduling of patient consultations and additional appointments for proton eligible patients. For those patients not eligible the Care Coordinator will work to triage the patient to the appropriate clinical specialist such as a Medical Oncologist.

The Care Coordinator will work closely with the clinical nurses and radiation oncologists to coordinate all referral information and subsequent scheduling of a consult at the proton center in a safe and timely manner. Additionally, the Care Coordinator will work in concert with the finance team to determine patient's insurance/financial eligibility and with the concierge team to assist with any needs the patient may have.

ESSENTIAL FUNCTIONS:

•Advancing inquiries who are candidates for proton therapy to a scheduled consultation as timely and safely as possible.

•Manage and optimize weekly consultation slots to serve patients.

•Answer and respond to phone, fax, and web inquiries in a prompt manner. (Phones immediately and fax or web inquiry response within 2 hours of business day)

•Interacts with referral sources, respecting HIPAA requirements, to begin to determine eligibility for proton therapy, with adherence to the protocols established by PCPTN and the radiation oncologists

•Able to speak with potential patients about the general clinical benefits of proton therapy

•Serves on Patient CARES Team, meeting regularly with the finance team and/or other appropriate personnel to exchange information on patient issues and to understand patient obstacles prior to scheduling consultation.

•Responsible for scheduling patient consultations and gathering appropriate medical records and imaging

•As required, presents cases to physician group for the purpose of confirming clinical eligibility for consultation.

•Utilizes available resources such as Patient Ambassadors and information packets to assist and educate prospective patients

•Follows up with patients still in the decision-making process

•Inputs demographic, insurance, marketing information, and clinical details with precision, thoroughness, and detail into appropriate databases.

•Establishes and maintains excellent patient-focused working relationships with partner hospital/physicians/referring physicians' office and facilities, and proactively communicates with same to address problems/improve quality of process.

QUALIFICATIONS AND REQUIREMENTS

•Bachelor's degree preferable

•Proficient with MS Office suite

•Able to implement policies, procedures, and computer applications related to the Patient Care Coordinator position
