We take pride in our creative and collaborative work environment and in delivering clinically excellent, trauma-informed, recovery-oriented services.What are we looking for in a Care Coordinator?In this role, the Health Home Care Coordinator works with individuals who have chronic health conditions and who are transitioning from inpatient stay or are high users of the healthcare system. Using excellent communication skills, the Care Coordinator will provide services and coordination to clients to ensure continuity of care across health and social service programs. This position requires strong interpersonal and organizational skills in order to build rapport with clients and to coordinate with various healthcare providers and community services. Comfort and confidence in addressing medical topics and in doing home visits is essential.
