Description

The Telehealth Clinical Pharmacist supports the operations and growth of clinical pharmacy services across the organization by providing telehealth patient visits. The Telehealth Clinical Pharmacist will assist the Clinical Pharmacy Director with planning and coordination of efforts of the multidisciplinary team providing comprehensive medication management services and chronic disease state education.

Requirements

Doctorate of Pharmacy from an accredited school of pharmacy

Completion of ASHP Accredited PGY1 Residency Program

Preferred but not required: Completion of ASHP Accredited PGY2 Residency in Ambulatory Care or relevant specialty
