Job Description Are you bilingual? Come join our dynamic Health Department team. CPR certification is a plus. Grade 15 General Statement of Duties Performs professional outreach duties and public health education; performs directly related work as required. Examples of Essential Work * Assists clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, and overcoming barriers to obtaining needed medical care and social services; * Performs Daily management of client caseloads include individuals with severe medical, behavioral and/or psychological health concerns requiring trauma informed practices to meet their complex health needs. This includes, but is not limited to, home visiting, care coordination and planning with other health and social service providers, and the neighborhood outreach to vulnerable populations. * Utilizes existing and new systems of coordination to ensure that clients are receiving and establishing the appropriate care and referrals to services/providers; * Assist with triaging phone calls, scheduling appointments, and interpretation and registration at community outreach clinics; * Supports client and general public communication, both written and verbal, in languages other than English; * Conducts neighborhood outreach to members of local businesses, schools, and residential homes; * Represents the Health Department at local health fairs, festivals, and other public venues to provide health education and share information regarding departmental services; * Provides education about benefits of preventive health services through workshops that are culturally and linguistically appropriate; * Works to identify language and low-literacy materials to use with clients and the general public to provide education about accessing preventive health services, navigating local supports provided by social service agencies, such a food and transportation assistance, and health and wellbeing; * Conducts home visits, as necessary/requested, to reach vulnerable populations; * Helps clients develop health management plans and goals; * Coaches clients in effective management of their acute and/or chronic health conditions and self-care; * Collects and maintains data relating to public health issues; * Provides needed information and demonstrations concerning how to perform certain work tasks to new employees in the same or similar class of positions; and more. Acceptable Experience and Training * Graduation from High School or GED; and * Some experience in health care or social services; or * Any equivalent combination of experience and training which provides the knowledge, skills and abilities necessary to perform the work. Required Special Qualifications * Bilingual skills in both English and Spanish * Valid New Hampshire Driver's License. * CPR/First Aid Certificate a plus Additional Information: This position is funded by the American Rescue Plan Act through December 2026. In partnership with the Police Department, the goal of the position will be to provide neighborhood outreach to assist residents in accessing health care and services to improve basic needs, as well as establish/coordinate neighborhood improvement projects. The position will be physically located at an office in the neighborhood that they serve. Manchester residents are strongly encouraged to apply.
